Modder here, it has to do with the way the cigarettes item is coded and how the game handles loot spawning. The Cigarettes item has a value called "count" assigned to it, with an assigned integer of 20. Basically this tells the game that whenever the conditions are met to spawn it, to spawn 20 of them at once.
